Dr. Amanda Holden continues the Five Pillars of Aging series with Pillar Four: skin quality. While wrinkles often get the most attention, many people who feel like they look older are actually seeing changes in texture, tone, brightness, and the way their skin reflects light.

Skin quality includes pigmentation, sun damage, brown spots, redness, pore size, texture, crepey skin, acne scars, fine lines, and overall radiance. When the surface of the skin becomes rougher or less even, the face can look older even when there are not many true wrinkles present.

This is also one of the most overwhelming areas of aesthetics because there are so many treatment options. Dr. Holden explains the difference between common tools like microneedling, RF microneedling, IPL, chemical peels, fractional lasers, and CO2 resurfacing. Each one has a different purpose. Microneedling helps stimulate collagen. RF microneedling adds heat energy for tightening and texture. IPL can help with color correction, including brown spots, redness, and sun damage. Chemical peels support renewal and can address concerns like brightness, pigmentation, acne, and melasma. Fractional lasers treat only a portion of the skin at a time to encourage remodeling, while CO2 resurfacing can create more dramatic improvements for deeper texture, wrinkles, and sun damage.

The biggest takeaway is that patients do not need to memorize every device name or understand every technical term. What matters most is having a provider who understands goals, skin type, Fitzpatrick skin type, lifestyle, downtime tolerance, and where the patient is in the aging process.

The best treatment is rarely just the newest treatment. It is the treatment that is right for the person, the skin, and the long-term plan.
